Dr Jeremy Sandford, GP and Clinical Lead at the CCG for Long Term Conditions would like to invite you to attend the focus group to discuss and help the CCG develop a long term conditions strategy for people in Camden. It would be great if you could confirm attendance to the above focus group which will be held at Camden Clinical Commissioning Group, Stephenson House, 75 Hampstead Road, London, NW1 2PL.
Patients and their carer’s are welcome to attend and we looking for people who have one or more of the following conditions.
- Diabetes
- Chronic Obstructive Pulmonary Disease (COPD)
- Chronic Kidney Disease (CKD)
- Hypertension
- Epilepsy
- Heart failure
- Atrial Fibrillation (AF)
- Stroke
- Neurological diseases
With knowledge of numbers I can assure that we have sufficient refreshments. Please also let me know if you have any special needs.
